To make sure all member functions that require it are marked `_LIBCPP_EXCLUDE_FROM_EXPLICIT_INSTANTIATION` I compared the output of `objdump --syms lib/libc++.1.0.dylib` before and after, ignoring addresses.

9d16cbc5c8 [libc++] Adds more forward declaration headers.
The module validation script of D144994 validate whether the contents of
an include match its module. An include is the set of files matching the
pattern:
- foo
- foo/*.
- __fwd/foo.h

Several declarations of the stream headers are in the header iosfwd.
This gives issue using the validation script. Adding iosfwd to the set
of matching files gives too many declarations. For example when
validating the fstream header it will pull in declarations of the
istream header. Instead if writing a set of filters the headers are
granularized into smaller headers containing the expected declarations.

4f15267d3d [libc++][NFC] Replace _LIBCPP_STD_VER > x with _LIBCPP_STD_VER >= x
This change is almost fully mechanical. The only interesting change is in `generate_feature_test_macro_components.py` to generate `_LIBCPP_STD_VER >=` instead. To avoid churn in the git-blame this commit should be added to the `.git-blame-ignore-revs` once committed.

2ae52326da [libc++] Towards a simpler extern template story in libc++
The flexibility around extern template instantiation declarations in
libc++ result in a very complicated model, especially when support for
slightly different configurations (like the debug mode or assertions
in the dylib) are taken into account. That results in unexpected bugs
like http://llvm.org/PR50534 (and there have been multiple similar
bugs in the past, notably around the debug mode).

This patch gets rid of the _LIBCPP_DISABLE_EXTERN_TEMPLATE knob, which
I don't think is fundamental. Indeed, the motivation for that knob was to
avoid taking a dependency on the library, however that can be done better
by linking against the static library instead. And in fact, some parts of
the headers will always depend on things defined in the library, which
defeats the original goal of _LIBCPP_DISABLE_EXTERN_TEMPLATE.

385cc25a53 [libc++] Ensure that all public C++ headers include <__assert>
This patch changes the requirement for getting the declaration of the
assertion handler from including <__assert> to including any public
C++ header of the library. Note that C compatibility headers are
excluded because we don't implement all the C headers ourselves --
some of them are taken straight from the C library, like assert.h.

It also adds a generated test to check it. Furthermore, this new
generated test is designed in a way that will make it possible to
replace almost all the existing test-generation scripts with this
system in upcoming patches.

e39095a32e [libc++] Define _LIBCPP_HAS_NO_PRAGMA_SYSTEM_HEADER whenever we enable warnings in the test suite
This should make CI consistent on all the compilers we support. Most of
this patch is working around various warnings emitted by GCC in our code
base, which are now being shown when we compile the tests.

After this patch, the whole test suite should be warning free on all
compilers we support and test, except for a few warnings on GCC that
we silence explicitly until we figure out the proper fix for them.

eb8650a757 [runtimes][NFC] Remove filenames at the top of the license notice
We've stopped doing it in libc++ for a while now because these names
would end up rotting as we move things around and copy/paste stuff.
This cleans up all the existing files so as to stop the spreading
as people copy-paste headers around.

f4c1258d56 [libc++] Add an option to disable wide character support in libc++
Some embedded platforms do not wish to support the C library functionality
for handling wchar_t because they have no use for it. It makes sense for
libc++ to work properly on those platforms, so this commit adds a carve-out
of functionality for wchar_t.

Unfortunately, unlike some other carve-outs (e.g. random device), this
patch touches several parts of the library. However, despite the wide
impact of this patch, I still think it is important to support this
configuration since it makes it much simpler to port libc++ to some
embedded platforms.

a8d1182f66 [libc++] Remove some _LIBCPP_CXX03_LANG from iostreams headers.
With the STL containers, I didn't enable move operations in C++03 mode
because that would change the overload resolution for things that today
are copy operations. With iostreams, though, the copy operations aren't
present at all, and so I see no problem with enabling move operations
even in (Clang's greatly extended) C++03 mode.

Clang's C++03 mode does not support delegating constructors.

317e92a3e8 [libc++] Enable explicit conversion operators, even in C++03 mode.
C++03 didn't support `explicit` conversion operators;
but Clang's C++03 mode does, as an extension, so we can use it.
This lets us make the conversion explicit in `std::function` (even in '03),
and remove some silly metaprogramming in `std::basic_ios`.

Drive-by improvements to the tests for these operators, in addition
to making sure all these tests also run in `c++03` mode.

ce565861c7 [libc++] Drop old workaround for iostreams instantiations missing from the dylib
On old Apple platforms (pre 10.9), we couldn't rely on the iostreams
explicit instantiations being part of the dylib. However, we don't
support back-deploying to such old deployment targets anymore, so the
workaround can be dropped.

c90dee1e90 [libc++] Re-apply fdc41e11f (LWG1203) without breaking the C++11 build
fdc41e11f was reverted in e46c1def5 because it broke the C++11 build.
We shouldn't be using enable_if_t in C++11, instead we must use
enable_if<...>::type.

This reverts commit fdc41e11f9687a50c97e2a59663bf2d541ff5489. It causes the
libcxx/modules/stds_include.sh.cpp test to fail with:
libcxx/include/ostream:1039:45: error: no template named 'enable_if_t'; did you mean 'enable_if'?

Still investigating what's causing this and reverting in the meantime to get
the bots green again.

fdc41e11f9 [libc++] Implement LWG1203
Libc++ had an issue where nonsensical code like

  decltype(std::stringstream{} << std::vector<int>{});

would compile, as long as you kept the expression inside decltype in
an unevaluated operand. This turned out to be that we didn't implement
LWG1203, which clarifies what we should do in that case.

396145d0da [libc++] Fix error flags and exceptions propagated from input stream operations
Summary:
This is a re-application of r357533 and r357531. They had been reverted
because we thought the commits broke the LLDB data formatters, but it
turns out this was because only r357531 had been included in the CI
run.

Before this patch, we would only ever throw an exception if the badbit
was set on the stream. The Standard is currently very unclear on how
exceptions should be propagated and what error flags should be set by
the input stream operations. This commit changes libc++ to behave under
a different (but valid) interpretation of the Standard. This interpretation
of the Standard matches what other implementations are doing.

This effectively implements the wording in p1264r0. It hasn't been voted
into the Standard yet, however there is wide agreement that the fix is
correct and it's just a matter of time before the fix is standardized.

57b08b0944 Update more file headers across all of the LLVM projects in the monorepo
to reflect the new license. These used slightly different spellings that
defeated my regular expressions.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.

Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

f7f5a1f778 [libcxx] Don't depend on availability markup to provide the streams in the dylib
Whether an explicit instantiation declaration should be provided is not
a matter of availability markup.

This problem is exemplified by the fact that some tests were incorrectly
marked as XFAIL when they should instead have been using the definition
of streams from the headers, and hence passing, and that, regardless of
whether visibility annotations are enabled.

That macro has been defined to _LIBCPP_HIDE_FROM_ABI_AFTER_V1 for many
weeks now, so we're actually replacing uses of it for uses of
_LIBCPP_HIDE_FROM_ABI_AFTER_V1 directly.

This should not change or break anything since the two macros are
100% equivalent, unless somebody is (incorrectly!) relying on
_LIBCPP_EXTERN_TEMPLATE_INLINE_VISIBILITY being defined.

8b6a5bf916 [libc++] Factor duplicate code into function templates
Summary:
The exact same code was replicated 11 times for implementing the basic_istream
input operators (those that don't use numeric_limits). The same code was also
duplicated twice for implementing the basic_istream input operators that take
numeric_limits into account.

This commit factors the common code into function templates to avoid
the duplication.
